(Non-profit organization with a 501(c)(3) tax-exempt status)

2. Organizations that provide education or social services

* Required Documents
– Completed “Grow Together” enrollment application & statement
– Proof of 501(c)(3) tax-exempt status

Application Deadline: Nov 30, 2016 (If mailed, it must be postmarked by Nov 30, 2016)
